NANDA Nursing Diagnosis Definition

Nanda nursing diagnosis « personal carelessness ” is defined as: constellation of culturally framed behaviors that imply one or more self -care activities in which there is a failure to maintain socially health and well -being Acceptable (Gibbons, Lauder and Ludwick, 2006).

Nursing activities focus on identifying the etiological factors of patient’s personal carelessness, determining the seriousness of the problem or facilitating the patient for efforts to face it.

Valuations

  • Perform a mental state exam
  • Perform a complete physical evaluation, paying special attention to personal hygiene
  • Assess the way of life and people available to help with daily activities
  • Perform a home security evaluation
  • Determine the level of participation and support of family members and other loved ones
  • Inquire about recent changes in the circumstances of life (for example, loss of a couple, physical changes)
  • Evaluate the patient’s desire and skills to change the situation
  • Monitor the indicators of personal carelessness resolution
  • (NIC) Support for self -care:
    • Monitor patient skills to perform self -care independently
    • Monitor the need of the patient for help devices for personal hygiene, dress, groom, eat and use the toilet

Patient and family education

  • (NIC) Support for self -care: teach parents or family to promote independence, when intervening only when the patient is unable to do things
  • Provide specific information to patient needs (for example, comment on diet requirements)

Collaboration activities

  • Create a multidisciplinary team, if possible (for example, who handles the case, nutritionist, doctor, occupational therapist)

  • Recommend alternative places for the patient, if a satisfactory level of home care is not achieved.
